OneCare Vermont

OneCare Vermont is a 501(c)(3) accountable care organization founded by The University of Vermont Medical Center and Dartmouth-Hitchcock Medical Center. The organization announced permanent closure to new business in 2025 while finalizing existing programs. Prior to closure, OneCare partnered with over 5,000 healthcare providers across Vermont to transform the state's healthcare system through population health management approaches.

The organization's model centered on three core business areas: network performance management, data and analytics, and payment reform. OneCare contracted with health insurance payers to shift from volume-based to value-based care delivery, providing providers with population health management tools, utilization data, and analytics to improve patient outcomes within fixed budgets. Their network included hospitals, primary care practices, specialty care providers, federally qualified health centers, skilled nursing facilities, home health agencies, mental health and substance use agencies, and area agencies on aging across Vermont's health service areas.

OneCare received IRS recognition as a 501(c)(3) organization in April 2021, reflecting its commitment to transparency and value-based reform efforts. The organization served diverse Vermont communities through care coordination that connected providers and helped identify at-risk patients for proactive outreach and resource connection.

Healthcare providers in Vermont chose OneCare Vermont when seeking to participate in accountable care arrangements and transition from fee-for-service to value-based payment models. The organization excelled in scenarios requiring coordinated care across multiple provider types, population health management tools, and data-driven insights to identify at-risk patients.

However, as of 2025, OneCare is permanently closed to new business. Previously, providers could expect support through analytics platforms, care coordination tools, payment reform expertise, and integration into a statewide network covering diverse care settings from acute hospitals to community-based mental health and aging services.
